Power of Attorney form - Once completed and given to Gallagher Transport Int'l., Power of Attorney gives Gallagher Tranport Int'l. the authority to sign all U.S. Customs documents on your behalf. This must be given to Gallagher Tranport Int'l. before an entry on your behalf would be submitted to U.S. Customs.


Instructions on how to fill out the Power of Attorney form - A form created to assist you in filling out our Power of Attorney form correctly.
Non-Resident Corporate Certification form - This form is required in addition to power of attorney for non U.S. Corporations.
Corporate Certification for Foreign Corporations form
GTI Pro forma Invoice & Packing List - A form provided to GTI customers in such case the importer was or has not been provided with a Commercial Invoice and/or Packing List.
Continuous Bond Application - A form needed for GTI to apply to customs for a continuous bond for a customer.
Chapter 52 Fabric Sheet - A form created to assist Gallagher Transport International find a classification for all fabric.
Single Country Declaration - A form declaring to Customs & Border Protection that your goods originated only out of a single country not multiple countries.
Multiple Country Declaration - A form declaring to Customs & Border Protection that your goods originate from multiple countries.
Quota Charge Statement - A form Customs & Border Protection requires anytime quota shipments are imported. This form is informing C.B.P. how the Export Quota was achieved and if any charges were paid or included in the commercial invoice price.
Toxic Substance Control Act - A form needed on importing goods that contain, or may contain, toxic materials. (etc. Glue, paint, industrial liquids, most chemicals.)

Customs & Border Protection CF3299 - Customs & Border Protection official form for the importation of personnel goods.


Supplemental Declaration CF3299

Customs & Border Protection CF4455 - An official Customs & Border Protection form needed when you are exporting goods with the intent to bring back to the U.S. without any changes.

Customs & Border Protection CF5106 - An official Customs & Border Protection form needed to certify address or to change address.

Customs & Border Protection form CF3311 - Customs & Border Protection official form for U.S. Goods being returned to the United States.

Fish & Wildlife Service
Fish & Wildlife Import Permit 3-200-20 form -
Fish & Wildlife requires a permit when importing hunting trophies. Most importers deal with a Taxidermist who has previous knowledge on what is needed by the U.S. Customs Broker. For those who do not, they must complete this form and send to Fish & Wildlife to obtain an import permit number.
