Power
of Attorney form - Once completed and given to Gallagher
Transport Int'l., Power of Attorney gives Gallagher Tranport Int'l.
the authority to sign all U.S. Customs documents on your behalf.
This must be given to Gallagher Tranport Int'l. before an entry
on your behalf would be submitted to U.S. Customs.

Single
Country Declaration - A form declaring to Customs & Border
Protection that your goods originated only out of a single country
not multiple countries.
Multiple
Country Declaration - A form declaring to Customs & Border
Protection that your goods originate from multiple countries.
Quota
Charge Statement - A form Customs & Border Protection
requires anytime quota shipments are imported. This form is informing
C.B.P. how the Export Quota was achieved and if any charges were paid
or included in the commercial invoice price.
Toxic
Substance Control Act - A form needed on importing goods
that contain, or may contain, toxic materials. (etc. Glue, paint,
industrial liquids, most chemicals.)

form for the importation of personnel goods.

Fish & Wildlife Service
Fish & Wildlife Import Permit 3-200-20 form - Fish
& Wildlife requires a permit when importing hunting trophies. Most
importers deal with a Taxidermist who has previous knowledge on what
is needed by the U.S. Customs Broker. For those who do not, they must
complete this form and send to Fish & Wildlife to obtain an import
permit number. This process must happen before customs clearance &
Fish & Wildlife clearance can happen. For more information please

Wildlife requires a designated port permit when importing hunting

United States Harmonized Tariff Schedule (HTSA) (2007) - An
actual copy of the Harmonized Tariff Schedule. Here you can look up
your classification and duty rate Customs & Border Protection will
impose on your inbound freight.
